Anthology Questionnaire

ComStar Media, LLC is a publishing company looking for authors to write original short stories for a group of anthologies. We are dedicated to offering great stories by unknown authors, giving them a foot in the door, as it were, of writing for publication. If you are interested in getting a short story published in an upcoming ComStar Media anthology, please fill out this questionnaire in its entirety and submit it to jenniferlee@comstar-media.com. Please do not send it as an attachment but as a part of the body of your email. Please make sure that your email is formatted in a way that will be easy to read and understand the important answers to these questions. Please use only one questionnaire per story and limit the number of questionnaires you send by five (5). For example, if you have three stories you want to send for consideration for an anthology, please send each of them in a different email with a whole new questionnaire, completely filled out. Do not use one form for all three stories.
